Intensive Care Coordination with Wraparound (ICCW)

Intensive Care Coordination with Wraparound (ICCW) provides service and support, information and referrals for your family based on a plan specifically designed to strengthen how you function as a family unit. The plan is created around the individual needs of your family.
 
ICCW is a process that works with your family to identify a support team of people you trust that builds on your family’s strengths to help meet your family’s needs.
 
Together with your team, we help your family become self-sufficient again. We will identify and help with issues relating to:

  • safety/crisis
  • family/parenting
  • health/medical
  • legal
  • emotional/psychological
  • social/recreational
  • residence/housing
  • spiritual/cultural
  • education/employment
  • transportation

Who is eligible for ICCW?

  • Youth ages 7-16 who have Severe Emotional Disturbance (SED), IDD or SMI
  • Youth who have SED and are participating in a Juvenile Justice service
  • Youth and their families involved with human service
    agencies who may need assistance coordinating the services
  • Youth with mental illness and/or multiple hospitalizations who are at risk for out-of-home placement
