Wraparound

Wraparound is a process with a goal to plan and implement services and supports for children and their families. Wraparound Facilitators help families develop a team comprised of the family, the supports of their choosing and involved agencies to create a plan of care. The team develops the plan based upon a global assessment of needs and strengths of the entire family. Wraparound facilitation occurs in the family’s home and community with a strong emphasis on family voice and choice.

Throughout the process, Wraparound staff will identify and help with issues relating to safety and crisis, family/parenting, health/medical, legal, emotional and psychological, social/recreational, residential/housing, spiritual/cultural, education and employment, and transportation.

Eligibility: Families involved in Children’s Protection Services and Prevention, Foster Care/Adoption and Delinquency Services with at least one child in the family involved (or has the potential to be involved) with multiple human service agencies.
