Intensive Care Coordination with Wraparound (ICCW)
Intensive Care Coordination with Wraparound (ICCW) provides service and support, information and referrals for your family based on a plan specifically designed to strengthen how you function as a family unit. The plan is created around the individual needs of your family.
ICCW is a process that works with your family to identify a support team of people you trust that builds on your family’s strengths to help meet your family’s needs.
Together with your team, we help your family become self-sufficient again. We will identify and help with issues relating to:
- safety/crisis
- family/parenting
- health/medical
- legal
- emotional/psychological
- social/recreational
- residence/housing
- spiritual/cultural
- education/employment
- transportation
Who is eligible for ICCW?
- Youth ages 7-16 who have Severe Emotional Disturbance (SED), IDD or SMI
- Youth who have SED and are participating in a Juvenile Justice service
- Youth and their families involved with human service
agencies who may need assistance coordinating the services - Youth with mental illness and/or multiple hospitalizations who are at risk for out-of-home placement
