Assertive Community Treatment (ACT)

This program offers adults with a severe mental illness assessment, individual supportive counseling, group therapy and family therapy as necessary. Intensive home/community based services are included as part of this service. Clients will also receive psychiatric assessments, medication reviews, medication assistance and nursing support services.

Medicaid and most other insurance are accepted; ability-to-pay based on a sliding fee scale.

 

Eligibility

Adults with a serious mental illness that requires the presence of a diagnosable mental, behavioral, or emotional disorder that exists or has existed during the past year for a period of time sufficient to meet diagnostic criteria specified in the most recent Diagnostic and Statistical Manual of Mental Disorders. The following disorders are included in this program only when they occur in conjunction with another diagnosable serious mental illness:

  • Substance abuse disorder
  • Developmental disorder
