CHILDREN'S
OUTPATIENT.
Through
Children's Outpatient, The Guidance Center offers mental health services for
children and their families including diagnosis, evaluation, and treatment.
Treatment may consist of individual, group, or family counseling,
information and referrals, case management, psychiatric evaluation and
medication management for children.
CHILDREN'S
OUTREACH (IN-HOME PROGRAM).
Through Children's Outreach, our professionals work with children who have
been diagnosed as being "severely emotionally disturbed" (SED), are at risk
of being removed from their homes, are returning to their homes after having
been removed, or who need mental health services but don't require
outpatient care. Services, including stabilization and case
management, are provided in the home setting. Training is also
provided for
parents to effectively address their child's disturbance.
CRISIS SCREENING. Our professionals provides timely
assessments and referrals for at-risk children who are experiencing acute,
chronic, or severe disturbances. Once a need is determined, services
such as intervention, case management or hospitalization may be arranged.

CASE MANAGEMENT.
When a child is having issues in two or more areas, a Case Manager will work
with the family to coordinate services for the child.
HOSPITAL LIAISON. A
hospital liaison assists a hospitalized child to receive services; and upon
discharge, helps him/her make the transition from an inpatient setting to
the community.
INTENSIVE SUPPORTIVE FOSTER CARE.
Through a collaboration with Vista
Maria and the Department of Human Services, The Guidance Center offers
in-home support and training to foster families. The goal is to
provide stable placement options for children to reduce the current reliance
on residential facility placement for Severely Emotionally Disturbed (SED)
children.

PARENT INFANT
PROGRAM (PIP).
The Parent Infant Program is a prevention and
intervention program that offers emotional support, developmental guidance,
advocacy and counseling to families with children up to three years old.
Services are provided through home visits to parents and children who are
identified as "at risk for poor attachment relationships" that could result
in developmental delays, abuse, and/or neglect. The primary goal is to
ensure the optimal development of the child.
WRAPAROUND.
This program provides individualized plan-based services and support,
information, and referrals to families with open
child-protection/foster care and delinquency cases, families open to
prevention services and post-adoptive services, families at risk for neglect
or abuse, families involved with many human service agencies who may need
assistance coordinating the services, children with mental illness and/or
multiple hospitalizations who are at risk for foster care or residential
placement, and/or families that have had contact with protective services
within the last 18 months. Throughout
the process, Wraparound staff will identify and help with issues relating to safety and crisis,
family/parenting, health/medical, legal,
emotional and psychological, social/recreational, residential/housing,
spiritual/cultural,
education
and employment, and transportation. |
